Dear families of Monroe School District student athletes,

We are excited to announce that we have partnered with FinalForms, an online forms, and data management service that will help streamline registration for student athletes and their families. 

FinalForms allows you to complete and sign athletic participation forms for your student(s), upload required documents, and view clearance status. The most exciting news is that FinalForms saves data from season to season and year to year, which means you won’t need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, which will save you time.

FinalForms launched today, June 1st, for the Fall 2023 sports season! You can register your account at: https://monroe-wa.finalforms.com. Please follow the prompts to create your account, add your students (use their school email address), and sign your forms. Moving forward, we will be asking that ALL guardians of student athletes use FinalForms.
