Our Community-Based Transition Program supports students ages 18–22 with active Individualized Education Plans (IEPs) as they prepare for life after high school. Through a combination of classroom learning and real-world community experiences, students build the skills needed for greater independence, employment, and community involvement.

The program provides personalized instruction to help students explore career interests, strengthen life skills, and plan for their future.

Students work on:

• Getting and keeping jobs
• Building independence in daily life
• Learning about personal finance
• Using public transportation in the community
• Connecting with community resources
• Exploring post-secondary education or training opportunties

The Community-Based Transitions Program is for students:

• With an active Individualized Education Plan (IEP)
• Age 18–22 who have completed four years of high school
• Recommended by their IEP team

Our team works closely with students, families, and caregivers to support each student’s individual strengths, goals, and interests. We focus on building confidence, encouraging self-determination, and helping students develop the skills they need to become active and contributing members of their community.
