Login OR create a new account on FinalForms to register your student for athletic participation.

Each student athlete participating in interscholastic activities is required to have or obtain medical insurance for expenses incurred as a result of injuries sustained while participating in extracurricular activity. Have your insurance and any other health information near you before you begin the registration process.

Please make sure to select one sport your student is interested in for each athletic season (Fall, Winter, Spring, Year-long). If you are unsure and do not know for future seasons, at the start of that season, log back in and add that sport prior to the the deadline for clearance.

If there is a change in circumstances (moving, medical or injury, educational enrollment) after June 1st, it is your responsibility to email the Athletic Department with details.

Financial Information and Assistance
- ASB Card, $50 - All students participating in a high school sport must purchase an ASB sticker. These funds directly support the year-to-year cost of the athletic programs, such as: facilities, equipment, uniforms, and more. The ASB sticker must be purchased prior to the start of the season in order to be cleared.
- The ASB card is good for the duration of the school year; pay once per school year, NOT per sport/season.
- Check or cash only accepted at the MHS ASB window. Credit and debit card payments can be made online through the Online Payment System.
- Please note you MUST pay the MHS ASB fee when registering for sports even if you go to an Alternative School, Private School, Running Start or Homeschool.
2. Athletic Fee, $110 per season - There is an athletic fee for each sport a student participates in.
- The athletic fee is non-refundable except for the following situations:
- Student is unable to play due to illness or injury prior to the first competition
- Student is unable to play due to family moving prior to the first competition.
- No refunds will be approved for students who are dropped for athletic code violations or who quit after the team's first contest. All refund requests must be received PRIOR to the end of the season.
3. Student accounts must be clear of all fines to participate.

Fee Waivers
If your student qualifies for free and reduced meals, fees associated with extracurricular activities will be automatically waived. This includes ASB cards, athletic participation fees, and other non-curricular activity participation. In order to receive these fee waivers, you must qualify for free and reduced price meals AND have submitted a Consent to Share Form, authorizing the school officials to view your eligibility status to waive fees. Monroe School District Family Event Pass
Family Event Passes are available for low-income families who qualify for Free or Reduced Meals with parent consent marked to share eligibility status. Family Event Passes allow eligible family members to receive reduced admission to ASB events held at MSD schools. Family passes are not accepted for playoff games or away games.
To apply for a Family Event Pass, please complete this Google Form. Please note: You must first apply for Free and Reduced Price Meals to be eligible for a Family Pass.
Physical - REQUIRED
In order to be eligible to play athletics at MHS, you must have a current physical on file.

Physicals are good for 2 years from the examination date and cannot expire during the season. You may print a physical form or pick one up in the MHS Athletics Office.

Doctor signature, date, and clinic stamp are required. Please review your form prior to submission to make sure they are complete!

Completed physical forms can be uploaded into FinalForms with your athletic registration.
- If you have turned in a paper physical form to the athletics office and need it to be uploaded into FinalForms, please send an email to tuiasosopoa@monroe.wednet.edu or call the athletics office at (360) 804-4511.
Running Start Students
Students are required to complete a WIAA contract if they attend Running Start full or part time.
Step 1: Complete the required WIAA Running Start Form
The Running Start contract must be filled out completely with all classes that you will be taking through Running Start (Term 1 thru 3) AND MHS (Semester 1 & 2 if you are not a full time Running Start student).
Step 2: Submit WIAA Running Start Form

This form must be signed by both the student and parent/guardian.

Then, email it and a copy of your Running Start schedule to your MHS counselor for them to review your schedule and sign.

Your MHS counselor will return it via email to you.

You will then forward it to the Athletic Office for Athletic Director and Principal signature.
Please be aware that all communication will need to be emailed.
Step 3: Grades
Grade checks are run throughout each sports season. Please provide a copy of previous semester and most recent quarter grades. Failure to provide grades may make you ineligible for competition.
If you take any classes outside the walls of MHS (attend any other school for ANY classes) you must provide the Athletic Office with your grades from that school.
Private, Online, Homeschool, & Alternative Learning Students - Sports only
Students who attend private schools, online schools, alternative learning experiences (ALEs), and homeschool are eligible to participate in athletic programs at their neighborhood school as a Sports Only Student. Per WIAA eligibility rules, students may participate in MHS athletics only if they meet one of the following criteria:
- They are enrolled at least part-time at Monroe High School (taking at least 4 classes), or
- They reside within the Monroe School District boundaries.
Students who are not enrolled part-time at Monroe High School and live outside of MSD boundaries are eligible to participate in athletics at their resident public high school.
If a student takes ANY class outside the walls of MHS they must complete one of the forms below ,in addition to registering for athletics via FinalForms.
Reminder: along with the MHS athletic participation fee ($110) for each sport, you will need to purchase an MHS ASB card ($50).
To register as a Sports Only Student:
All Sports Only Students must enroll/re-enroll with the Monroe School District each year they wish to participate. Visit the Register for School page for registration instructions. This step will give you access to complete the athletic registration in FinalForms.

Private, Online, and ALE Students: If you are enrolled in any off-campus classes, including Sky Valley Education Center, complete the Alternative Education and WIAA Eligibility form. This completed form can be uploaded into FinalForms during the athletic registration process or it can be returned to the MHS Athletic Office in person or via email (athletic secretary: maldonadoriverar@monroe.wednet.edu).You must also print your current class schedule, and return to the MHS Athletic Office. You will need to provide grades on grade check dates during the season in which your student is participating.

Homeschool Students: If you are homeschooled, complete the Home Based Education and WIAA Eligibility form. This completed form can be uploaded into FinalForms during the athletic registration process or it can be returned to the MHS Athletic Office in person or via email (athletic secretary: maldonadoriverar@monroe.wednet.edu).You must also print your current class schedule, and return to the MHS Athletic Office. You will need to provide grades on grade check dates during the season in which your student is participating.
