Who is Eligible for Benefits?
All employees who earn enough State Allocation to pay for mandatory benefits (dental, vision, life, and LTD) are eligible for benefits. However, employees must work a minimum of 17.5 hours per week to be eligible for Life insurance and Long‐Term Disability insurance. The following family members are eligible for medical, dental, vision, and voluntary life coverage:
- Your lawful spouse or domestic partner
- Your dependent children to age 26 -natural children, stepchildren,legally adopted children, children for whom you assume a legal obligation for
supporting anticipation of adoption (children of a domestic partner), or children specified in a court order or divorce decree.
When to Enroll
The annual open enrollment period runs from August 27, 2018 through September 28, 2018, for a November 1, 2018 effective date. Enrollment forms and online enrollment must be completed by the 15th of the month in order to be effective the 1st of the following month. Employees whose forms are not returned or online enrollment not completed within 30 days of employment or eligibility will be denied benefits and must wait until the next open enrollment period.
How to Make Changes
Unless you have a qualified change in status, you cannot make changes to the benefits you elect until the next open enrollment period. Common qualified changes in status include: marriage, divorce, legal separation, birth or adoption of a child, change in child’s dependent status, death of an enrolled family member, unpaid leave of absence for an employee or spouse, or if an employee or spouse has a significant change in employment status (part‐time to full‐time or vice versa). A new enrollment form or online enrollment must be completed within 30 days of the change in status (60 days for newborns and adoptions).
